Physical Disabilities

 

Home and Community Based Services (HCBS) Waiver for Individuals with Physical Disabilities

The Physical Disabilities Waiver serves individuals age 16 to 65 years of age who meet the criteria for nursing facility placement due to their physical disability, who are determined disabled by social security standards, and who are Medicaid eligible.

 Services Available on the Physical Disabilities Waiver:

Personal services: Assistance in completing tasks of daily living which  individuals would do themselves if they did not have a disability, including, but not limited to dressing, shopping, cooking, bathing, and other everyday tasks.

Assistive services: Medical equipment, home modifications and technology assistance devices which help the individual to remain in his or her home and increase his or her quality of life and level of independence.

Sleep Cycle Support: This service provides non-nursing physical assistance and/or nonphysical assistance supervision during the beneficiary’s normal sleeping hours in the beneficiary’s place of residence. This assistance includes: physical assistance or nonphysical assistance supervision with toileting, transferring, mobility, and prompting/reminding of medications. The provider will sleep and awaken as identified on the beneficiary’s Attendant Care Worksheet service plan and must provide the beneficiary with a mechanism to gain his or her attention or awaken him or her at anytime. The provider must be ready to call a physician, hospital, or other medical personnel should an emergency arise.

Personal Emergency Response Systems (PERS)and installation: an electronic device which enables certain individuals at high risk of institutionalization to secure help in an emergency. The individual may also wear a portable “help” button to allow for mobility. The system is connected to the person’s phone and programmed to signal a response center once the “help” button is activated.


In order to qualify for the Physical Disabilities Waiver, you must meet the following eligibility guidelines:
1. Be at least 16
2. Be determined disabled by the Social Security Administration.
3. Need assistance to perform normal rhythm of the day.
4. Meet Medicaid nursing facility threshold
