What is MFP?
Money Follows the Person is a federal demonstration grant intended to help residents of qualified institutional settings (Intermediate Care Facilities/Mental Retardation, Nursing Facilities) to move back into community settings.
The resident may receive up to $2,500 Transition Services to help with setting up their community residence.
Transition Coordination services are available to help the resident work toward moving into the community prior to receiving case management.
Case Management services can begin 60 days prior to the move back into the community.
The Home and Community Based Services (HCBS) waivered services will provide the in-home services to these individuals along with intensive case management
Whom does MFP serve?
This grant serves four populations:
- Frail Elderly (FE)
- Physically Disabled (PD)
- Traumatic Brain Injured (TBI)
- Mental Retardation/Developmental Disabilities (MR/DD)
What kind of services can the MFP customer receive?
HCBS waivered services specific to the waiver the individual would qualify for, plus any of the following:
- Transition Service - up to $2,500 start up cost
- Home modification/assistive technology services above the $7,500 lifetime cap
- Transition Coordination Service
- Targeted Case Manager Service
- Therapeutic Support (MR/DD & TBI only)
MFP consumers can receive 365 days of grant funded services, and may continue in the corresponding HCBS program if individual remains eligibile.
Where can MFP customers live?
Housing options:
- A home owned or leased by the individual or individual's relative
- A leased apartment
- MFP Qualified Assisted Living Facility (PD/FE only)
- A community based residence for no more than 4 unrelated individuals (MR/DD only)
What are the qualifications to be a MFP participant?
A current resident of a nursing facility (NF) or Intermediate Care Facility- Mental Retardation (ICF/MR), including State Mental Retardation Hospital (SMRH) with 90 days continuous stay
Medicaid eligible prior to receiving MFP services
Meets the functional eligibility for waivered services
Have interest in moving back to the community
